You can connect to your health records information at any time.

Our Patient Portal features include:

  • View and manage health records
  • Manage your profile
  • Manage documents and forms
  • Send secure messages to your provider
The patient portal isn't meant to handle emergencies. In an emergency, call 911 or visit the nearest emergency room.

How to create an account

  1. If you are a patient at CareArc, we’ve reserved an account just for you. All we need is your email address.
  2. Provide your email address at your appointment check-in and we will send you a secure login invitation via email.

Once you have provided your email address, log on using the following steps:

1. Check your email for activation link.

You will receive an email from after signing up at check-in. Please make sure to check your junk email.

2. Follow link to create an account

Click Sign Up and Connect. This will begin the account setup process. Complete the required fields on the next screens.

3. Welcome screen for MyHealthRecord™

Registration and connection are now complete. Congratulations! You can now access your medical record online.
